Hypothetically, if a priest was going to celebrate an Extraordinary Form Mass in a church without a communion rail, how would he handle the communion of the faithful? Would the faithful just kneel on the floor? Would they kneel in the first row of pews (assuming the pews have kneelers - and most churches do)?

Most churches at least have a kneeler/prie-dieu floating around somewhere. Put that at the front. If necessary, have servers move it into place at communion time.

Okay, let's say this hypothetical Mass is a wedding Mass. The only prie-dieu the parish has is the one for the bride and groom. Now what do you do?

I've been to Masses at which communicants have knelt in the absence of an altar rail or even a prie-dieu.

I'm sure many would be able to do this, but there are some people for whom that wouldn't really be a possibility (getting up again might be too difficult) and some people might forget to kneel if there's nothing for them to kneel on. In this hypothetical situation (which has some roots in reality - this is not a purely academic question), a lot of people attending the Mass would never have been to an EF Mass in their lives, and some wouldn't have been to one since before the Pauline Missal was promulgated.

ETA: That consideration does me give me another question. If a person (whether elderly or handicapped) has a problem kneeling (even with the communion rail) for holy communion, can they receive communion standing in the Extraordinary Form?

Judging from observation at my own parish: Yes.

My TLM Wedding Mass had no altar rail. My wife and I had portable kneelers, everyone else knelt on the first step of the sanctuary which had a long thick piece of carpet on it. Other portable kneelers are used during regular Sunday Masses as well as the piece of carpet.

Other Churches I have been to have taken the first pew, moved it a bit, and used it for kneeling and Holy Communion distribution.
